I use the following templates in my clinical practice and paste them into the blank form of the form_dictation and then modify the template as needed. This saves a lot of typing.

Normal Female Examination
Normal Male Examination

Like all template driven systems you need to take to modify these to suit your needs and remember to make all of the appropriate changes.
